The offical projections for Medicare spending follow current law. The Medicare actuaries produce "alternative illustrations" that show how those projections change under different assumptions. The first assumption is that the annual payment fixes for doctors continue to get made by Congress. The second is that the payment formulas created by the Affordable Care Act (ACA) prove too low, and Congess ultimately alters them to grow a bit faster than the economy.
